The United States Department of Agriculture (USDA) is creating 11 urban Farm Service Agency (FSA) offices as pilot programs in the United States with a focus on urban agriculture and the underserved populace. One of those FSA offices is slated to be located in Cook County, likely within the City of Chicago.

Nominate a Candidate for a FSA Committee
This form allows individuals to nominate themselves or any other person as a candidate. If additional forms are needed, this one may be copied or may be obtained at the County FSA Office.
